What Is Actually Happening Inside an Arthritic Joint
Wear and tear is a phrase that hides more than it reveals. Four separate things are changing, and they respond very differently to intervention.
Cartilage
The smooth, glassy layer capping the ends of the bones, a few millimetres thick, letting them glide almost frictionlessly. It has no blood supply and no nerves. It is nourished by diffusion from the fluid around it, which is one reason movement matters — that fluid gets pumped around by use.
Its lack of blood supply is also why it repairs so poorly, and why claims about rebuilding it deserve real scepticism. As it thins, the joint loses cushioning and the bone underneath takes more load.
Synovium
The membrane lining the joint capsule, which produces the lubricating fluid. In osteoarthritis it frequently becomes inflamed — for years this was thought to be a minor feature and it is now understood to be central.
That inflammation causes pain directly, and it releases enzymes that degrade cartilage further. It is also, unlike cartilage, well supplied with blood, which makes it reachable by anything circulating in the bloodstream. This is the target that anti-inflammatory supplements are actually aiming at, whether or not they say so.
Subchondral bone
The bone immediately beneath the cartilage. Under increased load it thickens and remodels, and bony spurs called osteophytes form at the joint margins. Some of the stiffness and lost range of movement comes from here rather than from cartilage loss.
The muscles around it
The part most people ignore and the part most amenable to change. Muscles absorb load before it reaches the joint. When pain reduces activity, they weaken, the joint carries more, and everything accelerates.
Why this matters for what you buy
Of the four, muscle is the one you can substantially change, and it is free. Cartilage is the one nothing taken by mouth has convincingly restored. Synovial inflammation sits in between — a genuine target, where the question is whether any given product delivers enough of anything to affect it.
What this means for supplement claims
"Rebuilds cartilage" runs into biology before it gets to evidence. "Supports a healthy inflammatory response" is at least aimed at something reachable. That difference is worth noticing on any product page you read.
