This ingredient never reaches the joint in any meaningful sense, and it is still the best decision on the label.

What it is

A branded, standardised black pepper fruit extract, typically around 95% piperine — the alkaloid responsible for pepper's heat.

Why a joint formula needs one

Because curcumin's journey to the joint has an obstacle right at the start. Taken alone, curcumin is poorly absorbed, rapidly metabolised and quickly cleared. Most of an oral dose never reaches circulation in useful quantity, which means it never gets anywhere near an inflamed synovium.

Piperine addresses that specifically, inhibiting the enzymes in gut and liver that would clear curcumin prematurely. The frequently cited 1998 study found roughly a twentyfold increase in bioavailability in human volunteers.

Credit where due

A turmeric joint product without an absorption strategy is discarding most of its own active ingredient before it can reach the tissue it is aimed at. Whoever formulated this understood that, and 5 mg is the standard commercial dose. This is the one figure on the panel sitting exactly where it should.

The caution that is not on the front of the bottle

Piperine does not enhance absorption selectively. It inhibits drug-metabolising enzymes generally, which means it can raise blood levels of a wide range of medications — effectively increasing your dose without you changing anything you swallow.

For most medicines that effect is modest. For drugs with a narrow margin between effective and harmful, it matters. If you take anything on prescription, this is worth raising with a pharmacist before starting any piperine-containing supplement.

Smaller points

It can irritate the stomach in sensitive people, which taking it with food generally settles. It may also add to the blood-sugar-lowering effect of diabetes medication.

The conclusion

Right ingredient, right amount, right reasoning — deployed to amplify a curcuminoid dose far below what the joint research used. Excellent delivery of a small payload. The ingredient earns its praise; the formula does not automatically inherit it.
