Sleep and Joint Pain Make Each Other Worse
The relationship between sleep and joint pain runs in both directions, which makes it one of the more useful things to address.
Pain disrupts sleep
Obviously enough. Discomfort makes it harder to fall asleep and causes waking during the night, particularly when turning over.
Poor sleep worsens pain
Less obviously, and more importantly. Sleep deprivation lowers pain thresholds — the same stimulus is experienced as more painful after a bad night. Research in this area consistently finds that poor sleep predicts worse pain the following day, and often more strongly than the reverse.
Why the direction matters
If poor sleep amplifies pain as much as pain disrupts sleep, then sleep is worth treating as a target in its own right rather than as a symptom that will resolve when the joint improves.
The wider effects
Poor sleep also reduces the energy and motivation for exercise, which is the intervention with the best evidence for the underlying problem. So it undermines the treatment as well as amplifying the symptom.
Practical things that help
- Positioning. A pillow between the knees for hip or knee pain, or under the knees when lying on your back, reduces strain at night. Simple and often surprisingly effective.
- Timing of pain relief. If you use painkillers, discussing timing with a doctor or pharmacist so cover extends through the night can help.
- Heat before bed. A warm bath or shower relaxes the tissues around a stiff joint.
- Daytime activity. Being appropriately active during the day improves sleep quality, which then reduces pain sensitivity. The virtuous version of the same loop.
- Consistent timing. Ordinary sleep hygiene applies here as much as anywhere.
When night pain means something else
Pain that consistently wakes you, or is present at rest rather than after use, is not typical of straightforward osteoarthritis and is worth mentioning to a doctor. The same applies to night pain accompanied by fever, weight loss or a joint that is hot and swollen.
